Build the orders, goals, and homebound statements for the 485 from the OASIS.

Open the POC Builder
The POC Builder is on the QA+ Review page, under the results lists. It shows on a SOC, Recert, or ROC OASIS only.

Run the review to open the OASIS on the QA+ Review page.
How to start the review - 2
Scroll down past the Errors, Warnings, and Alerts lists to the POC Builder box. Click the arrows on the right of the header to open or close it.

If the box says No POC, the 485 task does not exist yet in Kinnser. Create the 485 in Kinnser first. Then open the OASIS in QA+ again.
Check the summary
The top of the POC Builder shows key facts that QA+ pulled from the OASIS.

Check the Diagnosis list. It matches the diagnosis table on the Active Diagnosis page. To change it, go to that page.

Check the Frequency for each discipline, and the other items, for example walking ability (M1860), pressure ulcer risk, edema, oxygen, catheter, ostomy, dialysis, and PEG. QA+ shows only the items that apply to this patient.
Write the orders and goals
QA+ writes a first draft of the Orders and the Goals from the answers in the OASIS.

Read the Orders box. Change or add text as needed.

Read the Goals box. Change or add text as needed.
Get an AI review of the orders and goals
AI Review compares the Orders and Goals text with the chart and tells you what is missing.

Click AI Review. Wait while it says Reviewing.

Orders / Interventions and Goals each get a result: Looks good, Needs improvement, or Generated. Read the feedback under each one.

If there is a suggestion, click Use This Text to put it in the box. Then read the box and change what you need.
Looks good means there is no suggestion for that section.
Write the homebound certification statement
The Homebound Certification Statement box is at the bottom of the POC Builder. It holds the statement that goes on the 485.

Read the statement and change it as needed.

Click Remove Unused Services to take the services that have no frequency out of the statement.
Send it to the 485
Nothing goes to the 485 until you click Send To POC.

Click Send To POC. QA+ writes the Orders, the Goals, and the Homebound Certification Statement into the 485. "Saving..." shows while it works.

Click Switch to POC to open the 485 in Kinnser and check it. You can also click Go To, then 485.
Write the homebound reasons
The 485 also needs the two homebound reason statements. QA+ can review them or write them for you. The Homebound panel puts everything you need in one place.
- Homebound Status. A green chip says HOMEBOUND: YES. A grey chip says NO / NOT ANSWERED.
- Criteria checked. Which of the four criteria boxes (1a, 1b, 2a, 2b) are checked in the OASIS.
- Functional evidence (OASIS). The functional scores from the OASIS, with what each score means.
- Diagnoses. The diagnosis list. The primary diagnosis is marked Prim.
- Homebound section. The homebound part of the OASIS. Changes you make here save to the chart.

Click Homebound in the toolbar. The panel opens under the toolbar.

In the QA+ Homebound Reason AI card, click Review / Generate Reasons.
The same card is also on the Patient History and Prognosis page, under the homebound Specify fields.

Criteria One and Criteria Two each get a result: Looks good, Needs improvement, or Generated. Click Insert to add new text, or Replace to swap the old text for the suggestion.

Read the text in the field. Change it if needed.
Suggestions are drafts. Check them against the chart before you sign.
