Review an OASIS
Open an OASIS in QA+, work through the results, fix the problems, and leave comments.

Start from the Hotbox or QA Manager
QA+ adds a filter box and a green QA+ icon to the Hotbox and to the QA Manager.

Type in the QA+ Filter box to shorten the task list.
Quick choices: OASIS, DISCHARGE, ASSESSMENT, and 485. Click the red X to clear the box.

Click the green QA+ icon next to the OASIS you want to review.
Wait for QA+ to read the document
QA+ opens the OASIS on its own page, the QA+ Review page, and reads every page of the document.

A loading screen says "Reviewing OASIS...". Wait for it to finish.
A long document takes a little longer.
Get to know the screen
The QA+ Review page has three parts. For a map of every button and symbol, read The QA+ Review page guide.
- Page list (left). Every page of the OASIS. Click a page name to go to it. Click the small arrow next to a name to open that page by itself in a new tab.
- Toolbar (top). The Go To menu and the quick panels. See Use the toolbar panels below.
- Results (middle). The Errors, Warnings, and Alerts lists. The OASIS pages are below the lists.

Click Go To to jump to another place: OASIS menu, Episode, 485, Hotbox, QA manager, or Patient manager. Settings and Contact QA+ are at the bottom of the same menu.

Read the results
QA+ puts every result in one of three lists. The color tells you how serious it is.
- Errors (red). Fix these. An answer is missing, or two answers do not agree.
- Warnings (orange). Check these. They are likely problems, but not certain ones.
- Alerts (blue). For your information. Take a second look before you finish.
- BETA tag. The check is new. It may still change.

Start with the Errors list. It is open when the page loads. Each list header has a count and three buttons.

The page name on the right of each result tells you where the answer is in the OASIS.
Work through a result
Each result has three buttons and a page name.
- 1 · x. On the left edge. It appears when you hold your mouse over the row. Hides the result for this review. Click it again to bring the result back.
- 2 · Page name. Where the answer is in the OASIS.
- 3 · Eye. Shows the answers behind the result. You can change them right there.
- 4 · Clipboard. Adds the result text to the yellow comments box, with the page name. Click it again to remove it.

Medication Profile and Diagnoses (as evaluated) in this box are read-only. They show the data QA+ used for the check.

The form fields under them are live. Change an answer, then click the red Save button.
The grey bar that says No Changes Detected turns into the red Save button when you change something.
Errors on each page
Each OASIS page on the QA+ Review page has its own error list. It shows only the errors for that page.

Scroll to a page, or click its name in the page list. The page name is on a dark bar above the page.

Click the folder icon on the right of the page name bar. The error list for that page opens. Click the folder again to close it.

Each error has a clipboard icon. Click it to add the error to the comments box.
"No additional errors for this page at this time" means the page is clean.
Fix a problem
When you change an answer, QA+ saves it to Kinnser right away. You do not need to click Save on the form.

Change the answer on the OASIS page, or in the box under the result.
In the box under the result, the grey bar turns into a red Save button. Click it to save.

If a change does not save, a red "Change not saved" box appears at the bottom right. Click Retry save.
If it still does not save, open the OASIS again. You may need to log in to Kinnser again.
Pin a result to the top of the screen
A pinned result stays at the top while you scroll through the OASIS.

Click the pin icon on a list header. A bar with the first result appears at the top of the screen.

Use the arrows to move through the results one at a time. Click the red X to unpin.
Use the toolbar panels
The green toolbar has quick panels for common sections. You can change answers in a panel, and QA+ saves them.

Click a button: Vitals, Mental, Elimination, Cardiac, Respiratory, GG, Meds, Homebound, or Narrative.

Meds shows the medication list. As-needed drugs have a PRN tag.
The list is empty when the patient has no active medications for this visit.

Homebound shows the homebound status, the evidence for it, and the homebound section of the OASIS. It is covered in the plan of care guide.
Write the homebound reasons
Review the narrative
The Narrative panel shows the visit narrative with key words in bold. It can also write a summary, or suggest ways to improve the note.

Click Narrative in the toolbar.

On the Summary tab, click Generate Summary. You get a short clinical summary.

On the Critique tab, click Analyze Narrative. You get suggestions and questions.
Each suggestion has an Insert button. QA+ shows you the change before it goes into the note.
When you cannot change an answer
Two things can stop a change: a locked question, or a signed document.
- Grey question. Kinnser locks it because of another answer. Hold your mouse over it to see which answer locks it. An orange outline means the locked question still has an old answer in it. Change the answer that locks it, and the old answer clears.
- Signed OASIS. QA+ follows your signed-document setting. With Warn only, a yellow bar shows at the top and your changes still save. With Block edits, an Editing Blocked window opens and QA+ puts the old answer back.

To change the signed-document setting, click Go To, then Settings.
Settings guide
On a discharge OASIS
On a discharge OASIS, QA+ adds a Last OASIS Summary box under the results. It shows the diagnoses and other key answers from the patient's last OASIS, so you can compare them.

Click the arrows on the right of the Last OASIS Summary header to open or close the box.
Finish the review

When you are done, click Go To. Choose Hotbox, QA manager, or OASIS menu.
Leave comments for the clinician
The yellow COMMENTS box saves to the task comment in Kinnser.
Click the clipboard icon on a result to add it to the comments.
The speech-bubble icon on a list header adds every result in that list.
Type your own notes in the yellow COMMENTS box. QA+ saves them as you type.
